Sends a message to the chat the run answers. A run can send several. With Wait for the reply, the run then waits (an outside wait: no buttons, no live view) until the person writes again, and that message comes out as { message, sessionId }. That's how a flow asks a follow-up question in the same run. Without it, the item goes on unchanged.
